If your child requires a medical management/medical action plan in 2024, please ensure these are up to date and returned to the office for the start of Term 1.
If your child requires medication whilst at school, medications supplied to any DECYP school must be in date (not expired), and in the original containers or in a Webster-pak prepared by a pharmacist. If medications are expired, or not in the original containers or Webster-pak, we cannot accept these medications.
In addition, parents must complete an Authorisation for Administration of Student Medication form. In accordance with department policy, schools cannot administer ANY medication without these forms. FORM A is for non-prescribed medications (i.e. anti-histamines) and FORM B is for prescribed medications (i.e. Ritalin). Please ensure the correct form is completed. Forms can be found here.
Due to many students requiring medication whilst at school, if you are able to drop off your child’s medications and forms before the start of the term, we would be very grateful. This will allow staff time to process these forms and medications in a safe and organised manner.
